To obtain your Claims History Statement quickly, please prefer online consultation requests rather than make your request by mail. You can apply online with your Government Authentication Service account or with our web form

To obtain a copy of your Claims History Statement by mail, please follow the steps below.

Download, print and complete the Consultation and/or Verification Request form.
Remember to sign the form.

Append a legible photocopy of your driver's license (for identification purposes).

Mail all documents to GAA at the mailing address given on the form.
