To request in person a verification of the information in your Claims History Statement, please follow the steps below.

Please note that Claims History Statements can not be consulted/verified in person without an appointment.
Contact us by email at gaa-fcsa@gaa.qc.ca or by phone at 514-288-1537, extension 8, to book an appointment.
An agent will contact you within 48 hours.

On the day of your appointment, go to GAA’s office.
You'll be asked to complete the Consultation Request form and show your driver's license.

An agent will hand you a copy of your Claims History Statement. You’ll then have the opportunity to point out what information needs to be corrected.

You’ll receive a reply by mail.
