You can request a copy of your Claims History Statement from GAA.
A GAA agent will analyze your request, between 8:30 a.m. and 4:30 p.m., Monday to Friday.
We will inform you by e-mail on the next business day that your statement is available on line.

Complete the online Access Request form.
You will instantly access your claims file. Your reference number will be e-mailed to you.
You can make a follow-up request to access your file using your driver's license number and the reference number e-mailed to you.

Download, print and complete the form Access Request
Don't forget to sign the form.
Append a legible photocopy of your driver's license (for identification purposes).
Mail all documents to GAA at the mailing address given on the form.

Your Claims History Statement will be mailed to you within 20 days.

Go in person to GAA's office
(Monday to Friday, from 8:30 a.m. to 4:30 p.m.).
You'll be asked to complete the Access Request form and to show your driver's license.
An agent will then hand you your Claims History Statement.
If you notice an error after consulting your Claims History Statement, you can make a Rectification Request
