Verification Request by Mail

For requests to verify your Claims History Statement, we invite you to use our online services rather than make your request by mail.

To request by mail a verification of the information in your Claims History Statement, please follow the steps below.

If you already have a copy of your Claims History Statement, contact an agent at the number that appears on Statement summary.

The agent will note the information you wish to have verified and let you know how your request will be processed.

If you don’t have a Claims History Statement:

Download, print and complete the form Consultation and/or Verification Request form.

Append a legible photocopy of your driver's licence (for identification purposes).

Mail all documents to GAA to the mailing address given on the form.
