Verification Request in Person

To obtain your Claims History Statement the same day, we invite you to use the Online consultation with clicSÉQUR authentification or the Online consultation services rather than booking an appointment to obtain it in person.

To obtain a copy of your Claims History Statement and request a verification in person, please follow the steps below.

Please note that Claims History Statements can not be consulted/verified in person without an appointment.

Contact us by email at gaa-fcsa@gaa.qc.ca or by phone at 514-288-1537, extension 8, to book an appointment.

An agent will contact you within 48 hours.

On the day of your appointment, go to GAA’s office.

You'll be asked to complete the Consultation Request form and show your driver's license.

An agent will hand you a copy of your Claims History Statement. You’ll then have the opportunity to point out what information needs to be corrected.
